Where do I get a new form?
Forms are available from the UKvisas' website: www.ukvisas.gov.uk

Which form do I fill in?
There are eight standard forms. Please check the list below to make sure you fill in the correct form. Completing your Visa Application Form: You can either download a visa application form from this web link - Application Forms, or go to the VAC and obtain one, and then complete the form by hand OR you can download the form from the website www.ukvisas.gov.uk

How do I read / print these forms?
The forms are stored in the Adobe Portable Document format (PDF). To read them on screen and print them out you need to have the Adobe Acrobat Reader programme installed on your computer. You can download it free of charge from the link below:



Visa application forms are free of charge. If you have any problems downloading a form, please contact our Visa application centre.

Which form do I need?

You need to fill in only one of the forms below; depending on what type of visa you want to apply for. The guidance notes tell you which form you should fill in and also gives advice on what documents you need to give with your application.

Required Documentation:
The following documentation is mandatory:

  • Signed & Valid passport (Minimum Validity of 6 months from the date of submission of Application)
  • Completed and signed application form
  • A clear, passport sized (45mm x 35mm), colour photograph - please click here for exact details
